The journey begins at discharge. We know it can be a whirlwind as you’re excited to get home, and it’s easy to forget or miss important details in all the hustle and bustle. To ensure a smooth transition from the hospital to home, we highly recommend scheduling an appointment with Dr. Gonzalez and her wonderful team at Anna Health Family Medicine to set up a follow-up appointment after your discharge.

When you come in for your visit, please remember to bring a list of any new medications you’ve been prescribed and a copy of your discharge paperwork. We’re here to help you every step of the way!
